What Does NDIS Support Coordination Include?

Support Coordination is a capacity-building support funded by the NDIS. Our experienced coordinators will:

  • Explain your NDIS plan and funding categories
  • Identify your goals and match you with suitable providers
  • Coordinate and schedule your supports
  • Monitor progress and adjust services as needed
  • Support you with NDIS plan reviews and reports
  • Help you navigate the schedule of supports and service agreements
  • Coordinate your respite care and STA

The Three Levels of Support Coordination

1. Support Connection

For participants needing help to connect with providers and build their capacity to manage supports independently.

2. Coordination of Supports (Support Coordination)

For participants with more complex needs who require guidance, coordination, and troubleshooting across services.

3. Specialist Support Coordination

For participants with very high and complex needs, often involving crisis support or intensive coordination. Delivered by professionals with specialist expertise.

Who Can Access Support Coordination?

Not all NDIS plans include Support Coordination by default — but you can request it during your plan or plan review meeting. The NDIA determines funding based on your individual situation and level of need.
