What is Healthy Habits?
Feeling good is about finding balance, building confidence, and sharing the journey with others. Welcome to the Healthy Habits program, a fun and supportive 12-week
NDIS wellbeing program designed to help you build positive routines that last a lifetime. Whether you want to have more energy, learn to cook delicious, healthy food, or simply make new friends, this program is your first step.

Your 12-Week Journey to a Healthier You
Our program is structured in four simple phases, guiding you from setting your first goal to celebrating your long-term success.
Phase 1: Your Fresh Start (Weeks 1-2)
We begin by getting to know you and what you want to achieve, because your health journey is unique and your plan should be too.
Your Goals:
- Create a vision for your health and well-being.
- Set exciting and achievable personal goals.
- Get expert advice on food and exercise that’s right for you.
What You’ll Do:
- Have one-on-one chats with our friendly Dietitian and Exercise professionals.
- Join a fun group workshop to kick off your journey with new friends.
- Create a simple, personalised plan to get you started.
Feel-Good Outcome: A clear, inspiring plan that you feel motivated to follow.
Phase 2: Learning the Ropes (Weeks 3-6)
This is where we get practical, teaching you the simple skills to make healthy living easy and enjoyable.
Your Goals:
- Learn how to plan and cook simple, tasty, and healthy meals.
- Discover types of exercise you genuinely enjoy.
- Build your confidence every day in a supportive group.
What You’ll Do:
- Join group workshops on smart shopping and meal planning.
- Have fun in a hands-on healthy cooking class.
- Get moving with group personal training sessions designed for all fitness levels.
Feel-Good Outcome: The skills and know-how to make healthy choices with ease.
Phase 3: Taking it to the Community (Weeks 7-10)
We take our new skills and habits into the community for fun, social, and active experiences.
Your Goals:
- Join in fun, community-based activities like group walks or yoga in the park.
- Make lasting friendships with people who share your interests.
- Feel more confident accessing local parks, gyms, and community centres.
What You’ll Do:
- Explore local parks on organised group walks and activities.
- Try a new community fitness class with the support of the group.
- Discover local gems like community gardens or sports clubs.
Feel-Good Outcome: A strong sense of connection to your community and a new network of friends.
Phase 4: Making it Last (Weeks 11-12)
We wrap up by ensuring you have the tools and motivation to continue your healthy journey long after the program ends.
Your Goals:
- Create a personal plan to keep your healthy habits going strong.
- Connect with local clubs or groups you can join for the long term.
- Celebrate your amazing progress and success.
What You’ll Do:
- Attend workshops on staying motivated
- Create your own “Healthy for Life” plan.
- Celebrate your achievements at a final group event.
Feel-Good Outcome: The confidence and knowledge to maintain a healthy and happy lifestyle independently.
NDIS Funding Information
The Healthy Habits program is delivered as a Program of Support, providing a consistent and structured approach to help you achieve your goals. This program is primarily funded through the Core and Capacity Building – Improved Health and Wellbeing categories in your NDIS plan. We will work with you and your support coordinator to ensure the program aligns perfectly with your plan and goals.
